Clyde Buchanan is enjoying his retirement thanks to his commitment to twice-weekly Pilates sessions
Retirement initially did not exactly agree with Clyde Buchanan—at least, his body wasn’t so pleased with the switch from working every day to not.
“When I was working, it was there,” Buchanan, 67, says of his years-old back pain, “but it didn’t really bother me all that bad because I was active.”
That all changed when Buchanan, of Traverse City, retired in 2010 from working at the public library after 12 years. He decided to check with his doctor Stephen Andriese about what he could do to ease the pain of having spinal stenosis. (Spinal stenosis is a narrowing of the spinal column that causes pressure on the spinal cord, or narrowing of the openings—called neural foramina—where spinal nerves leave the spinal column.)
“He suggested yoga and he suggested Pilates,” says Buchanan, who quips he didn’t know much about the latter. “I thought it was something women do when they have nothing better to do.”
Today, Buchanan, who works with Pure Pilates instructor Kim Marian, acknowledges he has a much different view of Pilates.
“When I started there in August (of 2011), I was on a cane and it had gotten that bad—a heck of a lot of pain,” he says of his spinal stenosis. “Pilates, it’s worked great, and yes it is a surprise. I go a half hour twice a week.”
Buchanan says it wasn’t all smooth sailing once he got started with Pilates. Though he felt good about his exercise regimen with Kim, the back pain persisted to the point that he explored surgery. It was through this process, however, that he learned he had a heart murmur.
“I had to have a pre-op physical,” he says of receiving the news about his heart.
An open-heart surgery followed, leading to a break in his Pilates routine. But as tough as that successful surgery was to endure, Buchanan says he’s hopeful that being back at Pilates will ensure he won’t have to go through another surgery—for his spinal stenosis.
“Right now, I do not use a cane, because we have the core muscles all strengthened up,” he says. “I’m down to half a pain pill a day from four a day—I’m trying to wean myself off of these. What I’m working toward is not going back into that (back) surgery.”
“If I can keep this up and it keeps the pain down…then I may not need surgery,” he says. “It was scheduled and it would have gone off if I hadn’t of had the heart problem.”
As for retirement—this is actually his second retirement; the first one was in 1997, from his state of Michigan job—Buchanan is enjoying the down time, not to mention his improved health thanks to his time spent at the Pure Pilates studio. He’s now able to enjoy more fully not working.
“I go for a walk every day with my wife Dawn,” he says.

Spinal stenosis is closely associated with degenerative arthritis of the spine, and can be very difficult to live with.� As with many other back and neck conditions, diagnosing it early can help manage it successfully over time.
But the diagnosis process is not always straightforward.� According to Dr. Mark Ebell, Senior Editor of Essential Evidence Plus, a website that helps clinicians make evidence-based decisions, spinal stenosis “is a tough diagnosis to make”.
In an Essential Evidence Plus POEM, Dr. Ebell summarized and commented on a 2010 systematic review of medical literature that looked to what degree specific symptoms (age, bilateral leg or butt pain, pain in certain positions, etc.) correlated with a diagnosis of spinal stenosis.
Interestingly, the reviewers were only able to locate 4 studies (involving 741 �patients total).� Based on the fact that spinal arthritis is one of the most common types of arthritis (and arthritis is the leading cause of disability in the US), you would think the PTB would have arranged for more studies on diagnosing the stenosis that often results from it….
Anyway, the reviewers found that age (greater than 70 years), pain with walking, neurogenic claudication, bilateral butt or leg pain, unexpected urinary problems, no pain when sitting and pain relief when bending forward all correlated with a diagnosis of spinal stenosis, and most of them quite highly.� These signs and symptoms were obtained from the patients in the studies during the medical histories.� Their physical exams revealed that people who use a wide stance as their regular posture were more likely to have stenosis, as well.
This review also found that being younger than 60 and the absence of neurogenic claudication were correlated with not having spinal stenosis.

PITTSBURGH (KDKA) – Last year, 100 million work days were lost in the United States due to back pain alone.
If you or someone you know suffers from it, you know the pain can be debilitating and even devastating in some cases.
However, a new device may help to relieve back pain in some patients.
For Mary Ward, even simple activities like shopping or walking were nearly impossible because of a condition called lumbar stenosis.
“Mostly it was pain down the left leg. It would be very sharp and intense,” Ward said. “I couldn’t do my dishes without the pain being there and the longer you’re on your feet the more intense the pain becomes.”
The cause of the problem was in the bones of her lower back.
“She was active and relatively young. Her disc was relatively normal. It was only the joints in the back of the spine that were bad. But, she was very limited by her spinal stenosis and back pain from the bad joints,” AGH Neurosurgeon Dr. Donald Whiting said.
Lumbar stenosis is a when the tunnel through the spinal column in the lower back becomes narrow, most commonly, from wear and tear. Parts of the back bones get inflamed and thick, and the spaces for the spinal cord and for the nerves that go to the back and legs become pressed and pinched.
It’s actually a pretty common problem and many people have surgery to remove part of the back bones to make more room and release the pressure.
The procedure involves fusing the bones with rods and screws, to keep the spinal column strong. It does work, but there’s a drawback. It cuts down on the normal bending and flexibility the back is supposed to have.
That’s why doctors are studying an alternative to fusing the bones. It’s a device that mimics how the bones actually fit and move together, which is designed to relieve the pain of lumbar stenosis, while still maintaining motion in the lower back.
In the study, patients are chosen at random to get either the typical fusion or the experimental device. Then, doctors monitor them for motion and wear and tear above and below the device. Worldwide, there are only 100 patients with the device. Two of them are at Allegheny General Hospital.
“I was one of the first patients here getting it and we weren’t real sure what the risks were,” Ward said.
Ward had her surgery about two months ago.
“When I came out of surgery, I could immediately tell the difference,” Ward said. “It feels great and it did almost from day one.”
She still goes to therapy.
“I can bend over,” Ward said. “I’m doing my leg bends, crunches, and all the things that gave me problems before.”
However, she’s also doing all the things she loves.
“The ability at 40-years-old to be able to go dancing, go bowling, lift kids,” Ward said. “I can clean my house, do my dishes, I can walk and go to the store, I can go shopping if I want to.”
The device is currently only available through the research study.
One qualifying feature is that your disc, or the cushion of cartilage between the back bones, has to be normal.
If the disc is worn out or diseased as well, you will not be eligible.

Dave Mustaine, founder of Heavy Metal Rock band Megadeath, recently shared his struggle with disabling pain from severe spinal stenosis, herniated discs and bone spurs. His spine injuries caused him to suffer nerve damage in his left arm and hand. He ultimately underwent minimally invasive spine surgery to fuse a disc in his cervical spine.
In Mustaine’s interview with The Birmingham News, he joked that his mom had always warned him about the harm he would cause to himself with ‘head-banging’. In heavy metal rock performances, the artists engage in “head-banging” by standing with feet wide apart, and head lurching forcefully downward and upward, repeatedly. The effect is a wildly dramatic hair-whipping vision to behold!
He said, “Last summer during the Mayhem Tour, everything just went south… Something had broken in the back of my neck; I had a bone fragment in my spinal column the size of an eraser. The pain when I was moving my neck had me on my knees, sobbing. My son had never seen me this way. My hand went numb. My ring and pinky finger were not communicating with my brain. …Thankfully, the nerve damage he sustained was not permanent, and following minimally invasive surgery, he recovered full sensation.
Spinal stenosis, a narrowing of the spinal column, is usually a condition that occurs more often in those over 50 years of age, but can occur in younger people following trauma related spine injuries. Symptoms, which may be non-existent or develop gradually and worsen, include neck and back pain; pain shooting down the leg; and numbness, weakness or pain in the extremities.
To prevent future pain from herniated disc, bone spurs and spinal stenosis, after 30 years as a metal rocker, Mustaine says. “Now I move my torso a little bit more, and get the desired effect. I make the necessary adjustments I need to keep myself healthy. Really, it’s been a blessing in disguise.” Whether you are hoping to avoid minimally invasive surgery or not, here’s a lesson to keep in mind: always listen to your mother, she knows best!

Cervical Spinal Stenosis is a complex condition which occurs in the cervical region of your spine. It is where the narrowing of spinal canal happens and causes you to suffer painful symptoms. Myelopathy or spinal cord injury is characterized as a disturbance in your spinal cord, resulting to the loss of mobility and sensation. There are two main causes. One is trauma due to a fall, gunshots, war injuries, automobile accidents, and diving accidents.

Effects of the injury can differ depending on its level and type. It can be described as complete, which means there’s absolutely no function below the injury level. It means you cannot do any voluntary movements and that you lack physical sensation. They are normally bilateral, affecting both sides of your body. Incomplete injury means you can still have some sensation but incomplete. They can perhaps move one arm more than the other or acquire more function in either side.

You can combine activities like biking and walking with stretching and training for better results. Stretching and toning before exercise can decrease the wear and tear of your back. It also lessens risks of injury of muscles by increasing their flexibility.
It can also help if you main proper body mechanics. Be conscious in how you lift heavier objects, stand, sit, and even in your manner of sleeping. You can select a seat that can properly support both upper and lower backs. You may even put a pillow if necessary.
